Hospital to home care, is designed to provide support and assistance to individuals returning home after a hospital stay.

Returning home after a hospital stay can be challenging. Care and support can be especially helpful during the first days or weeks after discharge. This may include assistance with personal care, meals and snacks, domestic duties, laundry and medication support. Our person-centred care is tailored to individual needs.

The first 72 hours after a procedure or hospital stay can be an important and vulnerable time, often involving a new diagnosis and changes to daily routines, medication, meals or rehabilitation exercises.
