End OF Life Care

Through consultation with the doctor, a registered nurse will set up a plan and conduct it accordingly.

Advance care planning is important. It is a structured discussion with residents and their families or carers about their wishes and thought for the future. Advance care planning is key means of improving care for people nearing the end of like and enabling better planning and provision of care, to help them die in the place and the manner of their choosing. The main goal of life carer and be able to clarify people's wishes, needs and preferences and deliver care to meet these needs.

WHAT END OF LIFE CARE INVOLVES:

End of life care is support for people who are approaching death. It helps them to live as well as possible until they die, and to die with dignity. It also includes support for their family or carers. End of life care includes palliative care. If you have an incurable illness, palliative care will help make you as comfortable as possible by relieving pain and other distressing symptoms, while providing psychological, social, and spiritual support for you and your family or carer.

This is called a holistic approach to care, as it deals with the whole person rather than just one aspect of their care. Many healthcare professionals can be involved in providing end of life care, depending on needs. Hospital doctors and nurses, GP's, community nurses, hospice staff. People have the right to choose where they want to receive care and where they want to die. A palliative care team can provide end of life care to people and their families in hospitals, care homes, hospices and at home.

At NIGHTINGALE HOUSE we do provide end of life care, it is important to us to provide the care our residents deserve and want. We have a good relationship with the doctor and distinct nurses and them helping us provide good end of life care. We always aim to have necessary equipment in and end of life medication to give our residents the right to die pain free and with dignity and with families receiving the necessary support.
